INFORMATION ON PROCESSING PERSONAL DATA
&
CONSENT TO THE PROCESSING OF PERSONAL DATA

In compliance with the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as “the Regulation”).

I. – Basic provisions

I am informed that in connection with my registration to the UNIGOU Training Program organized by the Institute of Czech-Brazilian Academic Cooperation – INCBAC s.r.o., Jagellonská 1700/3, Prague 3, Czech Republic, ID No.: 04342909, (hereinafter referred to as the “Administrator”),

my following personal data will be processed by the administrator:

  • name(s) and surname(s);
  • date of birth;
  • sex;
  • home address;
  • e-mail address;
  • university of current/previous enrolment and graduation program;
  • dates of enrolment and graduation (if applicable) to the current/previous university program.

All the above listed personal data will be processed for the following purpose only, unless stated otherwise below in this consent:

  • arrangement of my participation in the UNIGOU Training Program organized by the Administrator and fulfilment of all the associated arrangements, as agreed in the Terms and Conditions of the UNIGOU Training Program.
I am aware that if I refuse to provide the above-mentioned personal data, my registration and consequently my participation in the UNIGOU Training Program cannot be realized.

The listed personal data will be kept only for the time required by law, i.e. time required for archiving official documents, unless stated otherwise below in this consent. The personal data will be accessed only by authorized personnel designated by the Administrator. Electronically stored personal data will be always secured by an access password; physically stored data will be stored only in secure repositories, so that only authorized personnel can access them. Internal regulation of the Administrator on the protection of personal data is freely accessible on the web pages of the Administrator (in Czech).

The processing of personal data is carried out primarily by the Administrator, unless stated otherwise below in this consent.

II. – UNIGOU Training Program Alumni

I am informed that following personal data will be processed and maintained for an additional purpose of maintaining the alumni base of the UNIGOU Training Program and further communication with the UNIGOU alumni students for the purpose of cultural and intergovernmental events related to the UNIGOU Program. These will be kept for maximally 10 years from the date when I cease my participation in the program:

  • name(s) and surname(s);
  • e-mail address.

III. – UNIGOU Training Program outcomes

I agree that the administrator can process also mine:

  • photos obtained during my participation in the UNIGOU Training Program;
  • video records obtained during my participation in the UNIGOU Training Program;
  • sound records obtained during my participation in the UNIGOU Training Program;
  • UNIGOU Training Program outcomes – applies only to the Article elaborated during the UNIGOU Training Program, as described in the Terms and Conditions of the UNIGOU Training Program;

for purposes of:

  • UNIGOU program and Administrator promotion (especially in promotional materials, flyers);
  • web presentation;
  • presentations on social networks (such as Facebook, Instagram, etc.);
  • presentation in other information materials and during events organized in connection to the UNIGOU Training Program.
I agree that these personal data will be processed and stored for the entire duration of the processing purpose (that is, even after I cease my participation in the UNIGOU Training Program). These personal data will be kept for maximally 10 years from the date when I cease my participation in the program.

IV. – Final provisions

I am aware of my right:

  • to have an access to my Personal Data (pursuant to Article 15 of the Regulation);
  • to require their correction (pursuant to Article 16 of the Regulation);
  • to deletion of personal data without undue delay, provided that the reasons under Article 17 of the Regulation are given;
  • to limit the processing of my personal data in cases under Article 18 of the Regulation;
  • to the portability of data in the cases provided for in Article 20 of the Regulation;
  • to revoke this consent pursuant to Article 7, paragraph 3 of the Regulation;
  • to file a complaint against the Administrator pursuant to Article 77 of the Regulation.
The processing of personal data is carried out primarily by the Administrator, but relevant personal data (namely: name(s) and surname(s); home address) can be processed for the Administrator also by the external processor: M. Smýkal, ID No.: 04984579 (accounting office).
I declare that I have carefully read this consent, understand it and agree with it. That I confirm online, in the related option of the Registration Form of the UNIGOU Training Program, that I am submitting within my registration for the UNIGOU Training Program.
